Physical Therapist

Department: Clinical - Therapists
Location: Dickson, TN

Now Hiring: Full-Time Physical Therapist (PT) – Home Health

Camden, TN & Surrounding Counties

Schedule: Monday–Friday | 8:00 AM – 4:30 PM- Regular weekday patient visits with participation in a rotating on-call schedule, including evenings and weekends. Daily hours vary based on patient needs, travel, and documentation.

Key Responsibilities

  • Evaluate patients and develop individualized treatment plans based on physician orders
  • Provide skilled physical therapy services in accordance with the physician’s plan of care and state regulations
  • Help patients improve mobility, strength, balance, and functional independence
  • Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques
  • Recommend adaptive equipment and home modifications to promote safety and independence
  • Collaborate with physicians and the interdisciplinary care team to achieve optimal patient outcomes
  • Complete accurate and timely clinical documentation and care plan updates
  • Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations
  • Encourage and motivate patients throughout their rehabilitation journey


Qualifications

  • Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
  • Current Physical Therapist (PT) license in the state of Tennessee
  • Home Health experience preferred, but not required
  • Two (2) years of experience in an acute care or similar clinical setting preferred
  • Current CPR and First Aid certification
  • Valid driver’s license, reliable transportation, and proof of automobile insurance
