What You’ll Do

  • Provide home health visits as assigned by the Clinical Services Manager, including patient assessment, admissions, care planning, implementation, and evaluation
  • Assess patients’ physical, psychological, social, and spiritual needs and coordinate care with the interdisciplinary team
  • Communicate with physicians and other healthcare professionals to ensure optimal patient care
  • Perform visits to evaluate patient needs and update plans of care accordingly
  • Deliver therapeutic, preventative, and rehabilitative nursing procedures as directed by the physician
  • Maintain accurate and timely documentation, including patient records, progress notes, and care plans
  • Provide clinical pronouncement of death and support families when appropriate to licensure

Qualifications

  • Current Registered Nurse (RN) license in the state of practice
  • Graduate of an accredited nursing program
  • At least one (1) year of clinical nursing experience (ICU, medical-surgical, or rehabilitation preferred)
  • Home health experience preferred
  • Current CPR certification
  • Reliable transportation, valid driver’s license, and current automobile insurance
