Are you a Social Worker (MSW) looking for a flexible role with purpose and real impact? At Tennessee Quality Care, we are seeking a compassionate Social Worker to join our Home Health team on a PRN basis.

What You’ll Do

  • Assess the psychosocial status of patients related to illness, environment, and support systems, and communicate findings to the care team
  • Conduct social evaluations and develop appropriate intervention plans
  • Maintain accurate clinical records, including notes and progress reports
  • Provide information, education, and referral services to patients and families regarding community resources and support services
  • Serve as a liaison between patients, families, and community agencies
  • Collaborate with interdisciplinary team members to support coordinated, patient-centered care
  • Assist physicians and care team members in understanding social and emotional factors impacting health outcomes
  • Participate in care planning, case conferences, and discharge planning
  • Build and maintain relationships with community resources and agencies

Qualifications

  • Master’s Degree in Social Work (MSW) required (Tennessee)
  • Current LMSW or LCSW licensure in the state of practice
  • Minimum of one (1) year of healthcare experience (home health preferred)
  • Strong communication, organizational, and interpersonal skills
  • Current CPR certification
  • Reliable transportation, valid driver’s license, and current automobile insurance
